At La Casa, Inc., we offer case management and/or service facilitation of Consumer-Directed Services to the DD, MR, and EDCD waiver recipients through the Division of Long Term Care Department of Medical Assistance Services. This assures continued appropriate and adequate services to all individuals. Activities may include counseling or training personal attendants or individuals regarding their responsibilities as an employer, making requests to KEPRO or Case Managers for adjustments to CSP/Plans of Care, and discussing needs for additional care for the individual and requests made. Consultant & Training Services

La Casa, Inc., is the largest provider of facilitation services in Virginia. We have been in business since March 12, 1984 as a training and resource company that provides the disabled community the quality care that they need. In 2001 we established Home Away from Home, Inc., and Comfort Zone LLC.
With these two companies we have been able to provide property management for individuals, and assist them in managing the provided group care.

Referrals* are accepted from individuals, Community Service Boards, Family Assessment Planning Teams, DSS, and Court Systems.

If you know of an individual who would benefit from these services, please contact the office nearest you. After initial information is taken, a qualified clinician will arrange a meeting with the referring agent and the family to further discuss service plan options.
